Suggest Treatment For Severe Vomiting And Blood In Urine In An Elderly Person

My wife has an 82 year old father who we believe for the past 3 months (in Hong Kong) has not been eating or drinking much at all. Currently, we have moved him to our home (in Los Angeles), and have been giving him electrolytes. He can't put down any solid food, and has been vomiting. If he lies down too long, he will vomit dark green liquid. If we have him sit, he will still vomit now and then, but light green liquid. He pees once or twice a day, but has small red dots in the pee. He has lost a lot of weight. Not sure what to do.

Hello.

Your wife's father needs urgent medical attention and hospitalisation.

His symptoms of vomiting greenish liquid are suggestive of bilious vomiting and the fact that he is peeing less and sometimes with streaks of blood are suggestive that slowly the kidney function has been decreasing.

He needs urgent medical attention and investigations like - a complete blood count to rule out any precipitating infection, liver function tests which may show obstructive jaundice, kidney functions tests, etc. He also needs and ultrasound of the abdomen to visualise the biliary tree, liver and other intrabdominal organs.

Treatment will then be according to the cause - but initially it will mostly include stabilising his general condition and correcting any electrolyte abnormalities by intravenous fluids, intravenous potassium, etc. He may need to be kept nil orally for sometime till he is able to tolerate food by mouth.
