Suggest Treatment For Severe Phlebitis In Legs

my father is 70 years old and has severe phlebitis in both legs and is on cumodin. he has open wounds on his feet/lower legs that are not healing. he has seen a wound care and an infectious disease specialist in the last two months and was treated with 3 rounds of antibiotics which have done nothing and his feet/legs are black/orange purple with scaly skin (he has had this for years) but now they are seeping, smell horrible and are getting worse. Today he finally told me he has two similar patches on the back of his upper thigh/buttocks on both legs that were bleeding profusely earlier today. the bleeding stopped but the patches look similar to his legs and have the same smell. I am concerned about gangrene. His BP was 100/60 which is low for him, which I assume if related to some of the blood loss. I cannot get him to go to the doctors. I am looking for any sort of possible diagnosis or suggestions. Thank you

My Opinion-

In My opinion, in the given situation you seem to suffer from-Chronic Venus Ulcers with Varicose Veins bleeding/from trauma by friction/and by incompetent long saphenous / short saphenous Veins which are varicose and are engorged from retrograde blood flow due to incompetent vein valves in long /short saphenous Veins / or its perforating communicating veins.
The black purple scaly skin with seepage for last 1 yr, suggests Chronic and long existing Varicose Vein in your legs.
Other causes need to be ruled out,after getting more details from you.

Do's Suggested-
Rest/Elevation to the legs.
Avoiding long standing work, for 3 weeks
Wound care properly from General Surgeon.
Evaluation of Leg Veins by vascular Surgeon,after getting USG scan with colour Doppler study of both legs,for comparative studies and remedies.
EVLT with Sclerotherapy as soon as possible,after control of infection of the wound in legs.
Supporting treatment from Vascular and General Surgeon would resolve your complex leg problem since last 1 yrs.

Next appointment-
Asap to review and plan the treatment for existing problem .
