Suggest Treatment For Severe Foot Pain In A Diabetic Person

I have been a Diabetic for 30 years. I have started on Long Wheat from June 1 this year. I was able to stop tab. Trajenta at lunch time because my blood sugar was dropping too low at lunch time. But my fasting and PP remain very high at 148 and 162. I had some blood tests done and found my Vit D to be very low at 6.8. When I check my sugar at home it is about 128-148 blood at the lab it is 171. I am on CetapinXR 1000mg BD, Amaryl 1 mg at BF, Voglibose 0.3 TDS, do you think it is time for me to change my medicines. I live in Gurgaon and there are no endocrinologists near by my home and I find traveling difficult because I get car sick. I have severe pain Ponnu my feet which makes it difficult to walk. What advise would you give?

General & Family Physician 's  Response
Hello and thanks for writing.

I can understand you concern and would try to help you in the best possible way. You fasting sugar levels are slightly elevated although PP levels are normal. It is advisable for you to get you HbA1C level checked for an estimate of last 3 months sugar. Drug modifications are not immediately required if your HbA1c levels are within targets. You may continue the same medications.

Your leg pain may be related to low vitamin d levels. You may begin with weekly calcirol satchets to get them within normal range. You may consult a nearby doctor for the prescription.

Also do not forget the dietary restriction and regular exercise as they are most important factor to control your sugars. Make a determined plan for it and follow it religiously. You will surely see a better glycemic control.
