Suggest Treatment For Severe Extensor Tone After Stroke

I have a patient who we are told had a stroke. she has severe extensor tone in her hips and legs making impossible to transfer her without a sling. Her upper extremities are hypotonic with almost alien hand like movements. Any ideas on how to help her be functional?

Do weight bearing as much as possible while keeping the alignment correct definately it will reduce spasticity.
