Suggest Treatment For Severe Back And Neck Pain

I am a 56 year old female. I have some serious concerns about treatment I received at a recent doctor visit. My family doctor sent me for routine bloodwork and my liver enzymes were high. She said she wanted to repeat the test in 4 weeks. When I had the bloodwork done again the liver enzymes were much higher. The ast more than doubled and was over 250. The alt was higher too but not as high as the other but it was still like 120. My family doctor then sent me for an ultrasound of my liver. The results were abnormal stating I have severe fatty liver disease and a possible blockage of the bile duct. I have been feeling really sick for the last couple of months. I have severe pain in the upper part of my right side that runs from under my rib cage around into my back. I have back issues so at first i attributed the back pain to that but it is not the same as the pain I usually have. I also have been very fatigued and have alot of nausea, especially after I eat. I am not big on going to the doctor other than my primary and pain management doctors mainly for my back and neck issues but it seems to me this is all related to the liver thing. If i was not feeling so sick it would not bother me as much but the side pain has been getting consistantly worse and I am having alot of trouble going to the bathroom and when i do go it makes the pain even worse. My family doctor refered me to a gastro and liver doctor. I saw him the other day and before he even examined me he stated that I only have non-alcoholic fatty liver disease and that it is nothing to be worried about. He told me that the treatment was for me to loose 1/4 of my body weight. I will admit that I am a little over weight right now. I became disabled in 2009 after a pulmonary embolism. I would like to loose some weight this just seems extreme to me. I was very thin my whole life but since I stopped working combined with menopause did not help. I weigh 143 so that would mean I would have to loose over 35 pounds. Never does he attribute my failing health to the liver issues or even offer any other type of treatment for it. I understand that this is not the worst kind of disease to have but i just cant to seem to feel any better and that is the only issue showing up on the lab and ultrasound. I have had a nagging pain in my side for years off and on but not to the extreme it is now. It just felt kinda like a side sticker or something. My main concern is that this doctor told me that he would not do a test that was recommended on the results of the ultrasound for the bile duct issue and told me that he is assuming that this issue with my liver has not been an ongoing issue so it cant be causing the problems I am having, I have pain exactly right where my liver is and it is not getting any better. What if he is wrong and it has actually been an ongoing issue that i was not aware of until now. He told me it could not actually be any more serious issues like cirrosis or cancer because they would take longer to show up. I am just very sick and concerned that I am not being treated for the proper issue, Can you please provide any type of opinion on what else i can do to make sure it isnt cancer or cirrosis already. Other than my back and neck pain issues I have never felt worse in my whole life. Thanks for listening.

General & Family Physician 's  Response
Hello,

Fatty liver can be due to many reasons, as your doctor said its non alcoholic fatty liver which is commonly seen in overweight people. The symptoms like fatigue, pain in right upper part of abdomen, enlarged liver are related to non alcoholic fatty liver.

Whenever a patient comes to me with similar issue, I advice her to maintain a healthy diet and always a healthy weight, do exercise to loose weight. Exercising is always beneficial.

For the back pain and neck pain if its not severe then it can be managed with local application of ointments. Sometimes pain-relieving medicines with your doctor’s advice can help. If it persists for long then MRI spine should be done to check whats the underlying cause behind it. With the results your doctor will advice what next treatment option is best for you.

Hope I have answered your query. Let me know if I can assist you further.
