Suggest Treatment For Sciatica

My doctor has mentioned cancel equine syndrome but we are not sure without test, plus I have had several severe attacks of sciatica lasting up to 12 days straight with no relief at all even with strong pain meds. I can only walk across my yard without my lower back just killing me and having to practically tell myself to take steps to get back to porch. But for last two weeks or so my mainly left but also right foot is so swollen they hurt. Would any of those be an ER situation that they might do a MRI or CT scan that my doc thinks I need through ER department because I have no insurance for him to just write me a prescription for either?

-Take muscle relaxants to ease muscle spasms.
-Take antidepressants for chronic low back pain.
-Take pain medications for more severe pain.
