I understand that you are concerned about your brother's illness and the relevant treatment. Now, the important thing to realize is that
Schizophrenia is a chronic mental illness and medication needs to be take regularly and for a long period.
As you have mentioned, poor compliance to medication can lead to relapse of symptoms and complications.
Unfortunately, persons with Schizophrenia lack insight or an understanding about their illness and so, they often keep discontinuing their medication.
In your brother's current scenario, I would advise the following:
1) Firstly, if there has been a severe relapse, the this has to be brought under control. And if he is not willing to take medication, then he may need in-patient treatment.
2) In the long term (after the acute phase of relapse is controlled), you can consider the option of long-acting depot injections, instead of tablets.
3) You need to make arrangements for a caretaker who will be able to monitor him and ensure that he is taking treatment regularly.
