Hello

Your findings may suggest complex ovarian cyst.Echoes in cyst may indicate its complex nature.
Complex cysts may be due to both benign or malignant reasons.
Generally complex ovarian cysts are due to benign causes and it may require follow up ultrasound only.
Up to 3-4 cm cysts respond well and it may disappear in follow up ultrasound.

It is important to exclude malignancy due to your age.
You was rightly suggested for tumour markers like CA-125 estimation. Its level is normal in your case.
You may only need follow up ultrasound.

Spotting may be due to fibroid. You may need definitive treatment for fibroid.
Its treatment is both medical and surgical depending upon clinical findings, family is completed or not etc.

Get well soon.
