Welcome to health care magic.

1.The possible causes in this case could be testicular inflammation - orchitis.
2.The next possibilities include back pain or nerve compressions give a radiating pain in similar way, other possibilities includes inguinal hernia.
3.I would recommend to get an ultrasound examination of scrotum - that will give a clear picture of the testis, chord, its parenchymal status, vascularity and presence of any lumps or lesions within the testis.
4.If you have back symptoms - start with back exercises, that will relieve the symptoms.
5.Inguinal defect also need to be ruled out as it does gives the similar picture - ultrasound with cough reflex will help to identify the defect.
6.After further investigations further treatment accordingly.

Good luck.
