1.The possible causes in this case could be
testicular inflammation -
orchitis.
2.The next possibilities include
back pain or nerve compressions give a
radiating pain in similar way, other possibilities includes inguinal hernia.

ultrasound examination of scrotum - that will give a clear picture of the testis, chord, its parenchymal status, vascularity and presence of any lumps or lesions within the testis.
