Suggest Treatment For Left Sided Abdominal Pain

My wife (51 year old Female) was diagnosed with diverticulosis several years ago and we have managed it very well with diet. Only one small flare up since she was first diagnosed 8-10 years ago. She had colonoscopy 5 years ago, and other than her diverticula all was fine. First week of July 2016 she started having an upset stomach, bloat, mild diarrhea and some pain, we didn t think much of it at the time, since I was having same issues as well, we assumed it was a bug or bad food. Mine got better but her s continued until July, 20, 2016, this year. I had to take my wife to the ER for pain in all four quadrants of the abdomen. She told the ER doctor it felt and was acting different than diverticulitis. After blood and urine tests, xrays of chest and abdomen, they told her she had gastritis, and her intestines were full of air which was causing her pain, gave a prescription of ranitidine and ondansetron and what foods to avoid and told to avoid Naproxen, Ibuprofen and use Tylenol. Her symptoms improved for about 3 days. Then she began to develop a pain in her lower abdomen, sometimes a feeling of burning, from left to right, frequent urination, still feeling like she needed to go more when finished. On July, 27, I once again had to take her to the ER, (just in case it may be relevant: at the same time I was also having some UT issues. Since we quit eating cakes, and sugary foods years ago I laid mine off on eating a cake. Mine finally cleared up and I have since felt fine). After blood work and a urine test they said she had a UTI, trace of bacteria in urine. Discontinued Ranitidine. She was sent home with a prescription of Nitrofurantoin and Phenazopyridine. She got better for about 3 days until pain began to return. On day 3-4 after the visit she began to develop violent tendencies, aggression and aggravation and we had to discontinue Phenazopyridine. The aggression and aggravation was gone after a few days. On August 3, we once again wound up in emergency room, for severe pain in her lower left abdomen and sometimes burning sensations in lower abdomen. After blood, urine work and ct (she was the 4th to receive ct for stomach pain within time we were there) we were told she had mild diverticulitis. After reviewing her drug allergies to fluoroquinolones, sulfa, aspirin, and Phenazopyridine she was sent home with a prescription for Augmentin. She began the recovery diet of liquids for a few days, etc, etc. On August, 11 she had a follow-up with her primary care provider, since she was still having some pain, she was prescribed Metronidazole to assist the Augmentin. The Metronidazole caused constant nausea, even to smells, sleeping so much she had to be awakened to take her medications. By August 13, at 2 a.m. she was waking up vomiting. On August 14 p.m she discontinued Metronidazole until she could consult with her doctor see if it was normal for this medication. On Aug 15 she left a message for her primary care Dr., who was out for a few days, asking if it was normal. No response. On August 25, back in the ER with Extreme lower left abdominal pain. Vitals were: Temp, 96.3 pulse, 90 Bp, elevated Given iv saline, nausea medication and pain medication. After urinary test, BUN-CREATININE and ct scan, said it was mild diverticulitis, not bad enough to admit. Gave iv ceftriaxone, and because she was still in pain, a dose of iv morphine (which only alleviated the pain), prescription for more Augmentin, Promethazine for nausea, hydrocodone for pain, and sent home. Told to follow the diverticulitis diet until she gets better. Upon leaving vitals were: Pulse, 75 Bp, normal August, 30 went to see primary care for follow up. Still having pain. Scheduled for GI consultation September 8 and told to return to ER if symptoms continue to worsen again. At the visit, told no bacteria cultures have been performed from urine samples taken. Addendum: All throughout, every time she eats and food gets to the colon it causes pain. Only thing which doesn t seem to make her nauseous to eat is bananas, canned peaches, scrambled eggs, but they seem to cause more pain when it gets to the colon than broths etc. Each visit to the ER her pain and symptoms have been worse than the previous time. Since this has began she has lost 15lbs, 6lbs in the last 4 days.

As first-line management, you can take analgesics like paracetamol or aceclofenac for pain relief.
If symptoms persist better to consult a general surgeon and plan for an ultrasound abdomen.
