Suggest Treatment For Jaw Pain Caused After Tooth Extraction

I am a 72 yr old female. I have heart rhythm problems and taking atenolol, diltiazem 180 day, Dronederone (Multaq 500), Warfarin (for Afib), and couple other meds that probably wouldn't be pertinent. I had 2 lower back molars extracted 8 days ago and due to long curved roots, was hard extractions. I do fine all day but late in the afternoon or early evening the pain starts. Last night was the worst as it started about 6:00 but was about 7:30 before I got home and took a 5-500 hydrocodon APAP. After 1 1/2 hrs pain still svere and took anaother one and started putting ice packs on jaw. this went on for 3 1/2 hrs to finally pain subsided. I called the Oral surgeon today and he said this is normal. He advised me to take 2 pills at once when it starts tonight. Eschemic pain in bone he said. Is there anything else I can do to ward this off ahead of time? I am fine all day long but this hits in the evening and I can't make any sense out of the timing.
Sometimes before supper, other times after. Also have lost 10 # now from this. He says this is normal. Do I need to lay down mid day or anything ahead of time to keep this from happening every night? thanks! Jacqueline Nikolish

Geriatrics Specialist 's  Response
Hi Jacqueline,
Thanks for asking.
Based on your query, my opinion is as follows.
1. Ischemic pain is very severe and taking hydrocodon should help in mild to moderate reduction in pain
2. Application of heat or pressure should help. Try to bite something, or gum pressure application will reduce pain.
3. It will last usually upto 10 days, and increases in severity from day 2 onwards and then reduces slowly and disappears by ten days.
4. Gentle massaging of gums and muscles over it will help. Also avoid hard food and over extending of mouth to allow jaw joints and muscles to relax.
Hope it helps.
Any further queries, happy to help again.
