Suggest Treatment For Immune Thrombocytopenic Purpura

My sister in law is suffering from I.T.P....in which she is suffering from a very less platlets count often less than 20 thousand in count.she has been treated in max hospital in Delhi and in B.H.U ,varanasi.but there is no any progress in her disease.what should we do?

Hello dear,
Understand your concern.
ITP is a autoimmune disease in which antibodies produce against platele lead to its dysfunction and bleeding occur.
Treatment are below:
1. Aim of treatment is to increase platelet count and to prevent relapse in future, because no cure for disease.
A. Drug therapy
1. First line drug: IV/Oral prednisolone: it facilitate homeostasis in blood vessels and reduce bleeding
2. Second choice: IV immunoglobin
3. Steroid sparing agent: Azathioprin

B. Platelet transfusion can be give if not good response against drug
Keep the platelet count > 50 X 10 3/ul

C. Surgery: last option
If after 6 months of drug therapy, if platelet count not increased 30000/ul
go for spleenectomy.
After surgery high chance of infection: so vaccine against H.inflenza and streptococcus bacteria should be given.
