Hello and welcome to HCM,
As you have mentioned that the bone injury is very old one, 30 years old.
First of all you need to get a fresh X-ray to know the current status of the bone injury.
In case there is bone defect, you need to consult an oral and maxillofacial surgeon for it.
Bone defects can be treated with bone grafting.
The associated skin defect also has to be managed.
That can be done in consultation with a plastic surgeon.
Infact both the specialist need to work in conjunction.

Thanks and take care
