Suggest treatment for herniated disc

I was diagnosed w/ Arnold Chiari Type 1 malformation w/ 4mm herniation in 2001 and diagnosed w/ a herniated L5/S1 4mm in 2013. I have undergone cortisone shots with minimal relief. Can you please tell me if the two can be related, and if I undergo surgery for the herniated disc, can it affect my Chiari

Hello,
Thanks for your query.
These symptoms are part of lumbar disc degeneration with PIVD where the nerves get compressed by the protruded disc.
There are several treatment modalities with medicines, physiotherapy and with injections in the back called epidural injections and root/facet blocks.
If you don’t get better with these treatment ,I suggest you undergo surgery, by which the nerves are decompressed by removing the protruded disc (discectomy), which would significantly improve your symptoms
