Suggest Treatment For Hematuria With Fatigue And Dizziness

I am an RN. My grandfather is 89, he has never had any type of testing or really been followed by an MD. His diet consists of chocolate, ensure, and cigarettes. He just retired 2 yrs ago from a lifetime of working in construction. About 4 yrs ago he started with UTI's, some hematuria on and off, frequency, etc.. We found a clinician willing to talk to him and treat him with cipro without doing any tests (as he refuses all testing, hospitalizations,blood work, etc).
He now is experiencing what he says looks like molasses when he voids-thick urine with blood. This lasted 3-4 days and now he is extremely fatigued, weak, dizzy, etc..I have enc fluids, promoted his taking the cipro he has on hand and enc foods high in iron. He basically has no appetite and I feel he is failing. He has not complained of pain, he refuses to go to the doctors and I don't what to do for him. Help me help him, please.

I am sorry to hear about your father who is having significant hematuria and now weakness. In my practice a patient in this situation needs to have blood work and imaging studies and possible admission to a hospital. This is a serious condition and needs complete evaluation.

From your description he may have a severe case of urinary tract infection with possible kidney infection and need IV antibiotics. In addition he sounds like he is severely dehydrated and again will need IV fluids to correct his fluid deficit.

My biggest concern is possible renal failure. Since he has had hematuria for years his kidneys could be damaged from what ever caused the hematuria and he is now destroyed his kidneys

I strongly urge that he seek medical attention immediately
