Suggest Treatment For Forearm Bruise

My son has an oblong bruise on his inside forearm that has been there about a month. The bruise is about 4 inches long and 2 inches wide with the inside 2 inches being clear skin with no bruising at all. There is no indication of an insect or spider bite. the duration of the bruise, size of the bruise and clear center worry me. He is 18 years old and in good health.
