Hello,
Thank you for asking at HCM.
I carefully read your question and understand your concern.
If you are bleeding so much with your periods you will need to find the cause of it first of all. Secondly it is very important that you do complete blood counts and take iron orally and sometimes IV if you are bleeding so much so to avoid transfusions as much as we can. What I would recommend is:
1. To find the cause of the bleeding you will need a vaginal ultrasound. it will see the thickness of the endometrium (inner uterine layer) and also check for uterine masses (mostly fibradenoma) which might cause heavy bleeding.
2. If you have not had a pap smear in the last 3 years you will need to have one done and gynecological visit with it
3. If the pap smear and the vaginal ultrasound and the vaginal visit are normal than the physical causes of the bleeding are ruled out and we might need to work on the hormonal causes (disfunctional uterine bleeding) or problems with the cloting system (Von willebrand)
4. First after normal tests I would recommend you to try combined oral contraception for 3 months- speak with your doctor to check if you have contraindications for those
5. If the bleeding is heavy even under the oral contraception than clotting problems might be considered. You may use tranexamic acid, Advil etc to help with bleeding. A good option might also be an intrauterine device (mirena) that might help with bleeding.
6. If none of the options above work and you are heavily bleeding and do not want to have any more children you might do a hysterectomy- it is the definite treatment if other options are not working.

Hope this is helpful and answers to your question.

Best regards,
I find this answer helpful
