Suggest Treatment For Diabetes

My age is 43 years, I am a sugar patient last 11 years, Now , I have some problem because my cretinine - 1.8 , urea - 50, sugar-154 (PP), 122(F), potasium-4.4, Please give me some advice for development from this condition.My weight is 59kg.

Thank you for the query.
As per the information provided, you are 43 yrs old (gender not mentioned), diabetic for the past 11 yrs (currently PPBS-154 mg%, FBS- 122 mg%) and laboratory investigations suggestive of renal impairment (Creatinine 1.8, Urea 50). You have mentioned you weight as 59 kg, but haven't given your height, so BMI cannot be calculated and your systolic BP is slightly above normal for your age (should be less than 140/90 mm of Hg)
Long term, uncontrolled Diabetes mellitus can lead to the development of a number of microvascular complications, and nephropathy, is one of them. It is recommended that you get your urine for spot ACR or 24 hrs urine for protein estimation done. If there is microalbuminuria/macroalbuminuria (albumin in urine), you need to start on ACE inhibitor/ ARB group of drugs (since your potassium is normal). This drug is going to help you out both for your Blood pressure and nephropathy. A USG of the kidney will reveal whether any chronic changes have developed in the kidney.
Presently, you are on Insulin 10 and 8 units (the type of insulin is not mentioned, I assume it to be of the premixed type). HbAic can indicate the control of your blood sugar over 3 months.
Dietary modifications with protein restriction (0.6-0.8gm/kg body weight) will help.
I would advise you to visit your treating doctor, since you need some investigations and a future management plan.
