Hi. Sorry to hear about the illness of the kid.
So far as per your description the source of fever may be a viral infection of nose producing symptoms of running nose, fever and possibly cough if posterior nasal drop occurs. Additionally cough may also be due to bacterial superinfection of the respiratory passages. In case of viral infection fever should be low grade. Incase of malaria/bacterial infection fever is usually high grade . Malarial antigen tests are not usually sensitive enough to exclude the disease altogather if tested negative. My personal opinion is if there is strong clinical suspician (high grade fever with rigor and chills along with no other obvious focus of such high grade fever), one should also give antimalarials. So my opinion is stick to the treatment advised. A 2-3 day regimn of antimalarial is not going to harm the child. Its better to treat rather than missing it because in many situations there may be co-infection (bacterial and malarial) as well.
Wishing you kid the best of health.
Regards.
