Suggest Treatment For Chronic Osteomylitis Of 4th And 5th Metatarsals

good day, I am a 25 year old male born with spina bifida, I had corective surgery at 9 months old and have had complications due to the disease such as decreased sensation in my legs even though i can walk, I have been dealing with a chronic ulcer in my foot for the pas 4 years, i live in the caribbean and I have seen many doctors who have treated the ulcer but neglected the underlying osteomylitis, now a recent xray has shown that i have chronic osteomylitis of the 4th and 5th metatarslals, i would not like to lose my entire limb nor both my toes which are healthy, I would like to know if the 4th and 5th metatarsal are removed if there are any orthopedic appliances that can be used to replace the 4th and 5th metatasrsal to save my toes?
