General & Family Physician 's  Response
Hello
You mentioned that you are having severe bleeding and a known case of hemorrhoids ( internal ) and bleeding is from one month .
According to you no medicine is effective , so in my opinion only one option is there ,get a surgery ( surgical removal is only an option ).
However , when such patient visit I prescribe them to take etamsylate +tranexamic acid combination 2 tablet twice in day until and unless contraindicated .
So consult your physician and take a prescription.
If no response within 2-3 day then get a surgery.
Good luck.
