hi, welcome to HCM,
recalcitrant acne vulgaris takes time .
fort type 3 and type 4 acne drug of choice is oral Isotretinoin cap once daily at night. take this medicine at the dose of 1 mg/kg body weight.
apptly topical adapalene at night in small amount and clindamycin at day time.

total treatment duration is 2-3 mth .
you can go for chemical peeling with 20% salicylic acid this is done by dermatologist it has very good result. some time comedone extraction is also required. you treating dermatologist will judge more precisely what is good for you.

hope this is helpful.
thanks
