Hello,
I understand the difficulty you and your wife are facing. As of now there is no cure for diabetes. However I feel that such wide fluctuations can be reduced. In my practice, if a patient develops such wide fluctuations, I change the mixtard. What I advise my patient would be a long acting insulin analog like Lantus at bed time and a short acting analog before each meal. You could discuss this with her doctor.
Ketoacidosis may be the cause of the abdominal pain if it is related to high blood sugar. Intense hunger occurring during low sugar period also may present as abdominal pain. There are also other rarer causes and I do not want to confuse you. The burning in the feet is due to neuropathy. Local applications containing capsaicin is helpful to many of my patients. If that does not give adequate relief I prescribe a medicine called Gabapentin. You could discuss this also with her doctor. There are other medicines also which would help her. So don't be disheartened.
If I can be of any help in the future, please do not hesitate to contact.
I wish your wife all the best.
Dr. Noble Zachariah M.D.
