General Surgeon 's  Response
Hi.
Thanks for your query and history, albeit short and specific.
Noted your finding of cordlike structure in the spermatic cord which needed voltarine injection.
Now it has returned.
The commonest causes are::
Inflammation of the structures of he cord like veins and lymphatics.
Extension of epididymutis.
Infected cyst of the cord.

I would advise the following:
Get a clinical evaluation and examination done by a General Surgeon or Ufologist.
Ultrasonography of the scrotum and abdomen.
Antibiotics and anti-inflammatory medicines till you are cured if infection is the issue.
Surgery if cyst or so. Or hernia.
I find this answer helpful
