Suggest Remedy For Epilepsy

I AM A DOCTOR OF MIDNAPUR COLLEGE WB AGED-54 YRS, HEIGHT- 5.7'' . WEIGHT- 80 KG .

PAST HISTORY:- 1) EPILEPSY- 1st ATTACK OCCUR IN THE YEAR 1969. DIAGONOSED BY THE DOCTORS AT DELHI , MILD FOCUS IN THE BRAIN.

2) SINCE 1963-1973,ABOUT 15-16 ATTACKS OCCURED.

3)BUT NOW THE SEIZURE IS UNDERCONTROL.

4) NO OLD CASE OF HYPERTENSION SINCE LAST 15 YRS AS BP LIE BETWEEN 160/100 mm mg .

5) LACUNAR INFRACT DETECTED BY CT SCAN SINCE LAST 7 MONTHS IN RIGHT MCA TERRITORY AND CEREBRAL ATROPHY.

*INVESTIGATION DONE - a) CT SCAN OF BRAIN

b)EEG- NORMAL REPORT

c)LIPID PROFILE & BLOOD SUGAR

REPORT NORMAL.

d) MRI, MRA & BILATERAL CAROTID

DROPPLER NOT DONE

* CHIEF COMPLAIN- FOCAL SENSORY SEIZURE ATTACK FREQUENTLY PERSIST FOR A FEW SECOND SINCE LAST 7 MONTHS.

* TREATMENT- 1)GARDINEL-60 mg TDS

2)EPTOIN - 100mg 3 TAB IN HS & 2 TAB AFTER

MEAL (TOTAL 5 TAB IN A DAY).

3)OXETOL - 600mg BD

4) TELVAS - H40/12.5 OD

5) ESLO- 2.5 1 TAB BD

6) ECOSPIN - 75 mg 1 TAB OD

7) ATORIIN -20 mg 1 TAB OD

AFTER THE STARTING OF EPTOIN 5 TAB /DAY , I AM BETTER THAN BEFORE. PLEASE GIVE YOUR VALUABLE OPINIONFROM NEUROLOGIST REGARDING TREATMENT , INVESTIGATION & OTHER ADVICE.

Hello,

Of course, more info on the prior type of seizures, EEG reports, past medications would be needed for more precise recommendations.

From what I can see I would reconsider the combination of drugs. Generally, it is preferred to combine drugs with different mechanism of action and Oxetol and Eptoin act in similar ways so I would gradually withdraw one of the two. If seizures are not improving then would consider adding another drug with a different mechanism of action like Levetiracetam.
