Suggest Remedies For Severe Sciatic Pain In Lower Back

I have fairly severe sciatic pain in lower back near backbone area on both sides. I am a female, 87 years old and physically very active -sports & exercise classes. Is it safe to use the hot, cold, hot tratment and if so, what should the timing (minutes) be for each part. Have massaged the area with my fingers and this has helped quite a lot so should I just carry on with the heat/cold scenario, and if so, about how many minutes for each part. Thanks you for any help you can give as I lead a fairly active life and hope to continue doing so if I can get the pain reduced. Sheila M.

General Surgeon 's  Response
Hello Sheila,
Welcome to Ask A Doctor services.
I have read your query and here is my advise.

* As per my clinical experience, severe sciatic pain with lower back
ache must be evaluated for increasing spinal cord sternosis,nerve
root problems affecting various UMN and PMN nerve segments.

* I would request you to get fresh MRI of the lower spine and pelvis ;
do follow up as you receive the soft copy of the results.
Till that time, assisted physiothapy , intake of better analgesic as
tramadol hydrochloride , local spray with menthol and diclogenic
ointments which help at controlling pain receptors of mu type.

Thank you.
Regards.
