HI
Well come to HCM
I really appreciate your concern, if this is the confirmed condition of anal fissure then surgery is the best option you have, what ever medicine you take that gives temporary relief but may not cure the disease.
The most important is underlying cause of formation of fissures and it is the constipation, I would advise to relieve the constipation first, try some fiber containing diet, drink more water, try some Isabgol powder, keep regular habit of defection, try sitz bath, such conservative line of treatment may helps you, else surgery is the cure
hope this information helps.
