Hi Dear !! Thanks for your query to HCM .
Read and reviewed your query and health concerns.
My opinion-
IN the given situation, You seems to be having-problem with Testes/Or spermatic cord-which needs to be checked physically and investigated and confirmed by your family Physician.
The reducing count indicates deteriorating Testicular function-which could be due to infection(STD-Gonorrhea) / or from high testes temperature, from overcongestion of blood(as in Varicocele of Spermatic Cord)

Consult another physician who would send you to Urologist and start corrective treatment.

You should not waste your valuable time to find out, Other causes of reducing sperm count - needs to be ruled out,by consults with Surgeon / and if needed from Urologist, before start of any treatment.

Do' Suggested-
Reply to-What to do to increase sperm count?
consult and check with family physician .
Get USG of Testes and LCr-blood test to confirm the diagnoses.
Consult Urologist, who would treat as per cause of reduction in sperm count.
Antibiotics x 2-6 weeks treats for Gonorrhea/Chlamydia-STD with prostatitis -if its found to cause reducing sperms.
Tab Neurobion-Fort-1-2 times a day x 5 days

If no relief further investigations and meticulous treatment and followups would be needed in your case.
Other causes of reducing sperms, needs to be verified and ruled out, before giving any treatment.

Hope this would help you to plan further of this complex illness of yours.
