Dear Friend.
Welcome to HCM. I am Dr Anshul Varshney. I understand your concern.
Yes, definitely you need adjustments.

With the charting done by you that i can see, you would require to reduce the dosage.

Following are my recommendations:
1. To take a combination of Hydrocholrthiazide and Amlodipine, commonly available as Amlovas-H 5/12.5 once a day.
2. Stop all other anti hypertensive medicines.
3. To take low salt and low fat diet
4. Exercise daily.

I would also suggest you to get following investigations:
1. Serum Creatinine
2. Lipid Profile
3. Fasting Blood Sugar
4. ECG
If you have any further query please ask me.
Stay Healthy.
