Suggest Diet For Pulmonary Hypertension

I am in the early stage of pulmonary hypertension, and was wondering if I could reverse it by diet and exercise ? I am 53 years old, blind, and 300 lbs !! I gained 60 lbs. over the past year while on disability. What do you suggest ? I am also type 2 ( insulin dependent), have hi blood pressure, high cholesterol and use a cpap machine for my sleep apnea.
