Hi,
Welcome to HCM
You are having Diabetes type II and Obesity, your treatment is on right line that is insulin and metformin and sitagliptin the need is to fine tune the dosage of insulin and planning of your diet.
Please take help of dietitian 25 K cal/kg body weight or even less as your activity is restricted. You can do carbohydrate count in diet and adjust dose of insulin accordingly with the help of your Physician guided by measurement of pre lunch and pre dinner RBS measurement, if you are not taking B12 supplement and multivitamin may also be added to your medication by doctor.
To reduce Post prandial blood sugar even another class of drug may be added known as AGI example is acarbose or Voglobose.
I am sure with little adjustment in diet and insulin dosage blood sugar can be managed in better way and monitored by FBS,PPBS and every three month by getting HBA1c level to keep
Wish you good life
Take care
