Dear friend, welcome to HCM!

This can not be due to tension !
When was the appendix operated?

Is there abdominal pain, fever?
Was the stool cultured? did the problem start after the operation?
He needs careful evaluation and treatment accordingly.
There are many differentials. pl. wait for colonoscopy findings to comment further.

Take care and please keep me informed of your progress.
Good Luck!
I hope to have answered your query satisfactorily.
Thank you!
