Suffer From CHF, Diabetes And Kidney Problems. Swollen Legs With Discoloration, Peeling Of Skin On Feet. Any Suggestions?

good evening, doctor. My brother has chf with ejection fraction of about 25% w/diabetes and kidney issues. His legs are very swollen with water and his skin looks like elephant texture with blackish tint at ankles and peeling skin on feet-however, he blatently refuses to elevate his legs and I am giving up on helping him. Your suggestions? he is very depressed but just refuses to do what his doctor tells him. He had quintuple bypass in 2007. Thanks.

Hi
It is understandable that your brother is undergoing a lot of mental trauma. All the above said conditions lead to pedal oedema. There is stagnation of blood and fluid in the dependent parts. In an ambulatory person as your brother this will be the legs.Longstanding venous congestion results. With time,Heamoglobin which is the blood pigment comes out and gets deposited in the skin.By oxidation and degradation it changes colour and becomes dark.Accordingly, the skin also appears dark. This skin is unhealthy, lacks proper nutrition. This is the reason for necrosis and peeling away of the skin.
Your brother is in urgent need of psychiatric help.Otherwise his physical condition will overwhelm his mental strength and he will lose his mind.He needs to brought out of this depressive state with the help of psychiatrist
