Hello I am Dr Ravinder Sharma ENT surgeon
On the basis of information provided it appears that you have a disease of pharynx and /or larynx(voice box). Alcohol and smoking are the risk factors for the cancer of pharynx and larynx. When used in combination they have a synnergistic effect(more harmful). I would advise you to
1. Stop alcohol and smoking immediately
2. Consult a ENT surgeon as you will need examination of esp. pharynx and larynx) ENT and may require endoscopic examination (pharyngoscopy and laryngoscopy). I will appropriate to discuss about the diagnosis when you have these tests done
Headache and its relation with standing up and light headedness can be because of posturral hypotension, or atherosclerotic plaque in your neck vessels. You will need a ENT and Physician consultation for this.

I hope this answers your concerns. Feel free to contact me if your need more information on this

With warm regards
Ravinder Sharma
