Smelly Discharge From Belly Button. No Improvement With Treatment. What Should Be Done?

My 21-year-old son has smelly discharge from his belly button. He has had it for 8 months and has been treat four times. His treatments have included oral antibiotics, cream antibiotics, and a powder to keep it dry. Nothing seems to be helping. Could this be something worse than a fungus or infection? What kind of doctor should he see? The doctor he has seen before if a general practice doctor. Thank you.
