Should Insulin Be Continued Alone Or With Other Drugs?

One month back I have undergone cbag at vijayawada,I am a diabitic patient since 10 years,during the surgery I have kept on insulin ( nova rapid)@ 15 units 3 times/day from date of surgery i am using the insulin alone now the fasting sugar level is 182,request your advce to continue the insulin alone are along with other oral anti diabetic drugs please advise me in this regard

I would require more information for a complete answer. The functional state of your kidneys is very important in diabetes treatment. You did not mention anything about them. If you are having kidney problems (low glomerular filtration rate - GFR) then pills can be troublesome. In any case insulin is the best way to achieve blood sugar targets. A "fast" insulin is not enough. You need a basal insulin as well. I would suggest insulins glargine or detemir. You also need a careful review of your diet, in order to make sure that you don't overeat but you also don't deprive your body from food for a long time during the day - if you do that, you're in danger of hypoglycemia.
The basal insulin will provide enough insulin to get you a normal fasting glucose and will cooperate with the fast-acting insulins (bolus) to correct your post-prandial glucose excursions. Adjusting the insulin dosage particularly in the first difficult days requires close cooperation with your doctor.
