Prolonged dizziness. Doing physical therapy. Done Epley Moneuver. How long will dizziness last?

I have had dizziness for 2 months. I was hospitalized for 3 days, all the tests have shown ok. The Neurologist said it is vestibular neuronitis. I have been patient waiting to get better. I went to visit Eye Dr and ENT Dr, they all told me they look ok too. Now I'm seeing physical therapy. The first visit to physical therapy she found out I do have BPPV because she saw my right eye was moving while lying down. So she correct it with Epley Moneuver. It has been one week, my dizziness still there. Can you assist with my problem? How long the dizziness can last? Any other ways can make my dizziness go away? Can the Moneuver be repeat? How can Physical therapist tell if Epley Moneuver works or not? Thank you so much!

Hello and welcome to HCM.

Thank you for your query.

The fact that you have been experiencing dizziness for this long, does suggest you could be suffering from vertigo.

The most common form of vertigo is BPPV (benign paroxysmal positional vertigo). In this condition, crystals are former in the semi-circular canal, which result in nystagmus (movement of the eye in the fashion the examiner described), vertigo (dizziness) in certain positions, etc.

The mainstay treatment for BPPV is the Epley maneuver. This maneuver is a very tricky one, and can only be performed by an experienced and well-trained individual; through which the crystals are removed out from their current location.

This might take more than one attempt, so yes, this can be repeated, and must be repeated till all the crystals can be removed.

The Epley maneuver is designed in such a way, that the crystals should be expelled, but as this can vary from person-to-person, we cannot be completely sure whether they were expelled or not. What most therapists do is, to wait and watch if the patients complaints subside within 24 hours, and if the patient reports to him/her again, the first attempt was not completely sucessful.

The dizziness will almost completely disappear immediately, provided your diagnosis of BPPV is accurate (and the cause for your vertigo) and the crystals are successfully expelled. In the meantime, you can request for medication, which patients with vertigo are usually treated (eg. betahistine hydrochloride). Please also make sure you do not make any swift head movements, and be slow while changing positions (supine to seated, standing to seated, etc.).

I hope I have succeeded in providing the information you were looking for. Please feel free to write back to me for any further clarifications at: . I would gladly help you.

Best wishes.

Hello, I had a right shoulder , elbow & wrist injury 1 1/2 years ago due to a fall on ice. I had surgery 4 months ago on the shoulder, some bone was removed and there was a small tear and fraying of my rotator cuff , which was able to be shaved or cut out without a major repair. Since then I have had 36 sessions of PT and my range of motion is excellent. I returned to work about 2 months ago and do mostly computer work and reviewing paper documents. I use a headset for phone calls, have the desk ergonomically set up by an ergonomic specialist and get up walk and do neck stretches about every 1 to 1 1/2 hours. About 6 weeks ago I having muscle spasms and tightness in my upper shoulder area and around the back right shoulder blade. About the same time I also becan to notice that if I moved a certain way I felt a little dizzy. I thought the dizziness was due to me holdong my breath at times when doing a difficult exercise. At PT they did soft tissue & tried to get the muscle to release & it wouldn t. I ended up getting muscle relaxers, eventually PT got the muscle to release a little, but this is an ongoing issue. Today they mentioned the trigger spot was in the levator area and explained to me how the muscle works. I came home to find out more about the Levator and some of the things I have notices, pain up to my ear, the spasming in my shoulder, the dizziness all make sense. Today was my last PT session (this was a work related injury). I wanted to give you a good history because I need to know how to explain this to the doctor correctly and what questions to ask. I have been living with pain for over a 1 1/2 years, so if this can help I would be very thankful.

Yes, After looking up my symptoms of dizziness (possibly Peripheral Vertigo, which I had some years ago; and it was corrected with a maneuver which I never knew the name of) I have found one name for one = the Epley maneuver. I need to find a Dr. in my area to do this. My ENT won t be back till July. I need help now.

I am 35 year old male.
Height is 5.8 and weight is 66kg.
I was diagnosed for Crohn’s in 2000. I had a ileal perforation in 2003 for which I was operated.
From past 3 months,I am having heaviness in eyes, slight dizziness(not rotational) as if I am drunk. I also feel exhaustion, fatigue throughout the day. I am not alert. If I walk for a short distance of 200mts also I feel very tired, dizzy and nausea. If I speak to a person I feel rest of the world around me is blank. All these symptoms are sever till 1-2pm and reduce as the day passes. When these symptoms started in mid of May 2011 my hemoglobin was 7 and ferritin was 1. So I was taking iron infusion every 15days. By end of July 2011 my hemoglobin was 11 and ferritin was 33. I was admitted to the hospital on 30th July as I had severe chills(shivering) and was there for 6 days. Every day I used to have 1-2 episodes of sever chills that would last for 1hour. But no fever. To investigate these chills and the above stated problems, all blood, urine tests are carried out and they are normal. Sugar levels and BP levels are also normal. As TSH was 6.59 doctor has increased the thyronorm(levothyroxine sodium) dosage to 50mcg from 25 mcg that I was currently taking from past 1.5 years. Chest X-Ray, ECG are normal. EEG and BAER test by neurologist are normal. CT scan of abdomen, pelvis and thorax was carried out. It showed presence at distal ileal loop thickening around 6mm. The neurologist has advised Betavert(betahistine) 16mg in the morning which I have been taking from past 3 weeks and Tryptomer 10mg-half tablet in the night. But I stopped it after 4days as it increased my drowsiness/heaviness in the eyes.

I did not have any episodes of chills after the discharge from hospital for the past 2 weeks.
However other symptoms have increased from past 2 weeks after discharge. 3months back I could go to office and but now I am not able to commute from past 3 weeks though able to work from home.
I got my eyes tested and consulted ENT as well and they are also normal.

I have been taking WALASA (mesalmine-400mg*2tablets) everyday from past 7 years for Crohns. I have stopped it from past one week as I doubt my problem may be due to side effect of this tablet or may be the tablet that is sold is fake.
I had similar symptoms during last year January 2010. I got admitted to the hospital and I was diagnosed for Thyroid. My TSH was 9.890. I started with 25mcg Thyronorm tablet. But heaviness in eyes, light dizziness continued for 4-5months and then disappeared. Brain MRI was also done in May 2010 and report was normal.

I get good sleep and there is no tension or worries. My appetite is also fine. I work from 12.30 to 9.30pm from past 5 years and I sleep around 1 in the night but I get up late at 9am. I consulted my doctors and nobody is able to diagnose my problem. So please can you let me know:

-what is the reason for these problems? Is this due to Crohns or Thyroid or something else?
-Is it the long term side effect of Mesalamine tablet?
-What is the cure for it? Please help..
