Popping and grinding sound in the knee. Pain after vigorous exercise. Need medical attention?

I am in soccer and rugby, and have had popping and grinding in my knee for years. and the usually pain came and went after vigorous exercise and games. But recently it has been constant pain. and the grinding is worse, I cannot walk any distance or go up stairs without it popping and extreme pain. I have been wearing a brace and taking pain meds. what should I do/ should I see a doctor
