Persistent Diarrhoea With Urine Coming From Anus. Cause For Condition?

I am a health care provider supporting a patient on ART who is not responding to treatment of diarrhoea despite using all measures as recommended by doctor and nurses. The patient self explained that diarhoea as urine coming from the anus , a condition which a general practitioner disputed since he is a man with no history of surgical intervention hence nobody ever thought that he may be having a vesico-rectal fistula . A period of four months with persistent diarhoea? What could have contributed to this condition?

Hi ! Your patient on ART may also be suffering from some kind of chronic inflammatory disease of the intestinal tract like Crohn's disease which might result in such internal fistula along with a history of persistent diarrhoea. The possibility of diverticulitis is also to be excluded in which this kind of fistula is not uncommon.I would suggest you to get him reviewed by a gastro-enterologist who may consider an endoscopy to diagnose the condition. That is an indication for extensive investigation – IV Urography / Lower GI Endoscopy / Ct scan of abdomen & Pelvis..
There are several possibilities – diseases of the bladder / bowel / others in vicinity infiltrating both...
If confirmed, surgery is the only option...
