Painful Sciatica In Legs, Extreme Back Pain, No Relief With Chiropractic, Physiotherapy, Steroid Injections. Surgery Required?

Is surgery required to repair a displaced S1 nerve root and flattening of the ventral aspect of the thecal sac? I have had painful sciatica in both legs for a year and half and extreme back pain, both affecting my quality of life . I have had chiaropractic therapy, physical therapy , steroid injections, and have taken pain meds on occasion, but no relief.
