Pain In Mid Back. Taken Chiropractic Treatment, Physiotherapy Treatment Earlier. Suggestion?
Hi I injured my mid back in Nov 2012 at work moving a heavy box. I have had chiropractic treatment, physiotherapy treatment and occupational rehab without improvement and left with significant mid back pain. I have had a CT scan. Findings report no significant compression fractures identified. There is bilateral facet arthropathy at the T7-8 level which results in minimal grade 1 anterolisthesis of T7 on T8. There is minimal disc space narrowing and vacuum phenomenon at the T8-9 level. Minimal facet arthropathy is also present at the T8-9 level. No significant spinal stenosis or neural foraminal narrowing is demonstrated within the thoracic spine. There is a tiny right sided disc protrusion at the T7-8 Level which does not cause spinal stenosis of neural foramina narrowing. I am left with significant pain 24 hrs a day and do not understand these findings, can you please explain? I am not currently receiving treatment and waiting to see if this can be treated. Facet arthropathy is usually seen due to trauma,degeneration due to aging,obesity,overuse and work related.The common symptom is persistent pain as the cartilage between the two facets of the adjoining vertebrae gets diseased.The vertebrae may slip in relation to each other causing spondylolesthesis or shift between the two vertebra in any direction,as there is a anterior shift between your T7-8.You would need to see a spinal surgeon /neurosurgeon who can assess and treat the problem.Use of NSAIDs,muscle relaxants, analgesics and steroids have been of use,Gabapentin has also been useful in some cases.Injection of steroids at the site of pain,Keyhole surgery are more aggressive options.
All the findings in the description suggests Degenerative changes in your lower dorsal spine causing pain.
* Use some support in the form of a brace * Avoid excessive bending activities * Use Analgesics & Muscle relaxants as needed * Ultrasound therapy & IFT may be helpful to some extent * Physiotherapy for strengthening the paravertebral muscles * Radio frequency ablation may be some help

Hi and welcome to HCM Your MRI finding suggestive of degenerative disc disease and arthritic changes & it occurs due to repetitive overload or stress to the disc & it increases the risk of disc herniation. The medications will relieve pain and help muscles to relax. Physiotherapy shall also help a lot, especially exercises that strengthen your back. Kindly note that exercises should only be done when it doesn't hurt. Adequate rest and proper nutrition is equally important. Drink lots of fluids since dehydration affects the disc. Eat a diet rich in fresh fruits and green leafy vegetables, nuts, milk, eggs, pulses and cereals. Drink 1-2 litres of water daily. Do brisk walking exercise for 40 minutes daily. Get 6-8 hours of refreshing sleep daily. Hope this will helps you. Take care.
