Hello,
The cardiovascular risks are multiplied many fold if person is having diabetes with hypertension and hypercholesteremia and specially if they are uncontrolled and long standing.
Symptoms of your father demand an assessment of his cardiac status by consulting a cardiologist and getting appropriate investigations,as early as possible.
Timely intervention would spare him of future complications.
Thanks

Hi,
A couple of differential diagnosis may be considered.DM+HTN+Dyslipidemia suggests a very high chance for atherosclerosis that is deposition of cholesterol within the wall of the blood vessels leading to occlusion of the blood vessel. This can occur in all the arteries. The most important of them is the coronary arteries that is the blood vessels supplying the heart.Occlusion of these vessels will lead to inadequate blood supply to the heart. There will be an imbalance between demand and supply. When the cardiac muscle does not receive the amount of blood it wants, there is hypoxia and hence there is pain. This is angina. This is what your father may be experiencing.Heart attack occurs at an advanced stage.Your father will require further evaluation in the form echocardiogram,TMT and possibly also a Coronary angiogram. The earlier the better for these tests.
Another possibility is occlusion of the arteries supplying the brain, and compression of the nerves supplying the arm like cervical spondylosis. This can cause all the symptoms described by you including dizziness and light headedness. But still my first differential diagnosis would be coronary artery disease with chronic stable angina.Hence detailed evaluation is a must.
