From your history of smoking and surgery for suspected gall bladder malignancy 4 weeks ago, the first thing one has to exclude is either a primary or a metastatic malignant lesion in your lungs. This might have come to light by the way of investigation for pneumonia and presumably were not present at the time of pre-operative investigations for your gall bladder surgery. You have not mentioned about the hitopathological report of the gall bladder specimen of yours. Whatever it is, if I were your doctor, I would have advised you for a CT/MRI scan of both the abdomen and the chest to reach nearer to the possibilities of these nodules. As your doctor advised you to see a pulmonologist, he may also consider an endoscopic biopsy of the nudules which has not subsided during the 3 weeks treatment for your pneumonia. The management will depend on this report finally.
