Not able to control urine is known as urinary incontinence.One can understand the discomfort faced due to it and the anxiety and social problems associated with that,particularly in a young person. First of all it has to be understood that the process of urination is controlled right from the brain,which controls the urinary bladder and the sphincters(muscular structures which encircle the outlet of the bladder and control the outflow of the urine and allow the urine to flow only when bladder is adequately filled and these are also partially under voluntary control of the person). So any problem right from the brain(like decreased mental status),in nerves which go to the bladder(due to injury),in bladder itself(due to infection etc ),in sphincters ,in prostate(a gland which encircles the bladder outflow tract in males and if it enlarges it causes obstruction to the urine flow and the bladder over flows leading to incontinence) or due to a fistula(abnormal outflow tract) incontinence of urine may occur. I recommend you to visit a urologist who is a specialist in dealing this problem.He will take complete history and perform a physical examination and perform certain investigations such as complete urine examination,an ultra sound scan, urodynamic study(to know the pressures in the bladder),x ray, if required ,apart from few other tests. Based on all these he can come to know the cause and treat accordingly may be with medicines,physiotherapy and surgery if required.
