Hi thanks for posting your concern in the HCM.
Considering your symptoms, I think this may be due to
Gastroesophageal reflux disease or dyspepsia.
Acute coronary syndrome or an anginal pain should also be ruled out. Although lower
respiratory tract infection or pneumonia does not usually present with burning sensation, this should also be ruled out. The back pain may also be due to any spinal abnormality or anxiety disorder. Detailed clinical examination coupled with Investigations are needed to rule out the possibilities.
My recommendation is you should consult your local physician for clinical examination. You may need the following tests-
ECG- you should do this at the earliest
Trop-T test to rule out Acute myocardial infarction
Complete blood count, ESR, urea, creatinine, LFT
USG whole abdomen
Chest Xray- PA
You may have Cap Rabeprazole+
Levosulpiride once daily before breakfast.
Please avoid tobacco, alcohol, carbonated beverages, spices.
Maintain good oral hydration
For any further questions, please write back.

Dr. Kaushik
