i would advise you to take him to a doctor.
it looks like an infection of the nail bed.
if allowed it will spread fast. it needs to be corrected with antibiotics.

the pain in the left big toe might be due to a infection of the nail bed .so u have to consult a physician , if the nail not viable excision of nail to be required with few antibiotics.
