Kid, Suffering From Abdominal Pain, Difficulty In Breathing, Loss Of Appetite. Normal Chest X Ray, Blood Tests. Any Ideas?

hi my son is 6 and is suffering with abdominal pain this last 2 weeks around his belly button and below,he also has to catch his breath as he says he cant breathe he vomited twice in the first week he is not eating properly he wants to eat but feels the food wont go down and feels like choking he has had a chest x ray and blood test for metabolic acidicios and all clear he is pale black circles on eyes and hardly and energy im thinking he might have a hernia I don't know as docs don't know

Firstly I would like to know several things before we can come to a definitive conclusion.

1. If he has had regular bowel movements. Does he remain constipated.
2. When was the last time he had a bowel movement, and what was the consistency- hard/soft/liquidy.
3. Any fever. 4. Has he been on any specific medicines in the past. 5. Is he currently taking any medicines like ibuprofen/advil/motrin etc.
5. Does he have any history of allergies/eczema/asthma. Or any family history of the same.

Based on the above answers, there could be several possibilities.
1. If he has history of constipation in the past, then often kids are not able to eat well and suffer from abdominal pain and vomiting especially if they are impacted.
2. If there is a strong family or personal history of allergies/asthma, and he is complaining of pain or difficulty while swallowing alongwith his abdominal pain, then eosinophilic esophagitis may be one consideration.
3. If he has been taking medicines like ibuprofen for a long duration especially on empty stomach, this can cause erosive gastritis which can be the reason for his pain.

I would suggest that he see a gastroenterologist if his symptoms are not improving and to evaluate for the above conditions and for further management especially if he has not been able to swallow food. The specialist may recommend endoscopy of his upper intestine if he feels the need to do so.
