Is There Any Doctor Available To Interpret Urine And Scan Report?

Hi Sir,
My brother is 11 years old and he had undergone a surgery 1 1/2 years ago due to hydronephrosis and grossly dialated left ureter and pelvicalyceal system. The procedure undergone was "left uretic tailoring(Hendren's technique) with left uretic reimplantation (6 fr DJ stent).
He was fine after that and did not have any pain and was normal.
Now the problem is, he's getting continous fever from past 1 month and the doctor advised to go for blood, urine and scanning tests, but in the recent scanning it showed "gross Hydronephrosis with thinning of renal paranchyma on left- sequel of chronic obstructive uropathy(resudual/ recurrent PUJ obstruction).
Please tell me the exact meaning of this statement in the report.
My query is will he be undergoing surgery again?
why wasn't it cured in the previous surgery.?
Is there any treatment only through medicines i.e without a surgery?

Please guide me, I'm so much worried about my brother's condition.

Thanks for writing in.

The statement means that there is ballooning of the left kidney with loss of kidney tissue which is a consequence of long term obstruction to flow of urine out of the left kidney. It is possible that this problem existed before the earlier surgery and since it was not reversible it is still seen at present. The clinical significance is that such a problem can cause infection and fever in the patient. If the right kidney is functioning normally then there is no need to worry.
