Is Surgery Advisable For Ovarian Cysts?

Hi Doctor, I have my irregular periods. This time it came on 10-10-2017 and lasted up to 25-10-2017. Got blood test and tsh test and they are normal. But in ultrasound reports we found that I have cysts in both the ovaries. 4.0*2.6 in one and 2.8*2 in other. Doctor prescribed me with the tablet regestrone 5mg. Will my cysts get removed automatically with the tablet or it need to be removed by surgery. We are planning for kid.. Please suggest.

Hello and welcome,

Irregular, prolonged periods and several cysts on ultrasound is highly suggestive of polycistic ovarian syndrome (PCOS).
The cysts have no indications to be surgically removed. In PCOS patients have low progesterone, large amounts of estrogen and testosterone. High testosterone level in the body may cause excessive hair growth, acne, considerable weight gain, absent, irregular or prolonged periods and infertility.
To confirm the diagnosis of PCOS, luteinizing hormone (LH) follicle stimulating hormones (FSH) and LH/FSH ratio should be checked. You may need some extra tests ordered by your doctor (glucose tolerance test, etc).
The treatment consists of life style changes (weight loss, exercises) and medications. If you wish pregnancy your doctor may prescribe pergonal or clomiphene (to induce ovulation) and metformin (decrease circulating androgens). Spironolactone reduce the symptoms of hirsutism, and prescribed only if woman doesn't wish pregnancy and usually used in combination with oral contraceptive pills.
