Hello and welcome to HCM,
A leiomyosarcoma stage 1b can be managed by surgical removal of uterus.
A follow up has to be kept to look for any evidence of metastasis or distant spread.
Removal of ovaries and fallopian tubes is not required.
Removal of appendix along with surgical removal of the uterus may be due to appendicitis or some other disorder of appendix.
Spread to leiomyosarcoma to appendix is unlikely since the tumor is stage 1 b only.
Spread to distant site and distant organs makes a stage of 4 and not 1.

Thanks and take care
